Physician Assistant/Family Nurse Practitioner - Dubois, WY

A Physician’s Assistant/Family Nurse Practitioner is a licensed health care provider, trained to provide direct delivery of patient care services. In accordance with the rules and regulations set forth by the State of Wyoming, a Physician’s Assistant/Family Nurse Practitioner will engage in the provision of outpatient, primary care services in a clinic setting as well as possible service in other locations such as nursing homes or other health care facilities. Physician’s Assistant/Family Nurse Practitioner must provide open access to documentation and formal supervision time for their supervising Physician.

Employee shall work up to 40 hours per week. The practice model may be evaluated every quarter by senior staff for purposes of clarification of practice, objectives and goals. Based on organizational need the employee may be required to work in areas other than that for which the employee was hired. Employee subject to mandatory performance expectations and requirements set forth in the provider contract. This provider job description is representative of the provider contract and superseded by the signed formal contract.

Dubois is located at the base of the Wind River Mountain Range. It is the perfect place for the outdoor enthusiast. There is an abundance of fishing, hiking and exploring. Come and practice medicine in a very special, rural, beautiful part of Wyoming.

CHCCW will provide, at its sole expense, a 2-bedroom, 1009 square foot unfurnished apartment, never occupied, for the period of one-year. This includes the water, heat, and electricity. Storage space will soon be available at this new complex for tenant convenience. If, at the end of the year the employee desires to continue to stay in the apartment the lease arrangement will be transferred to the employee at the monthly rate as currently structured by the landlord. This space will be available by December 1, 2023.

Salary Range is $110,000.00 to $130,000.00 DOE
