Position Summary:

Provide guidance and support to individuals impacted by Alzheimer's disease and other dementia. Respond to disease related questions/situations by providing guidance, problem-solving, decision-making support, crisis assistance, and connections to local services. Conduct assessments and action plans that identify needs and offer interventions based on those needs, including assistance with future planning, and in-depth education about the disease and caregiving. Responsible for implementing new and established programs and services with a strong emphasis on reaching new, underrepresented communities with volunteer-delivered programs.

This is a grant funded position.

Responsibilities

Essential functions and responsibilities include, but are not limited to:

  • Ability to handle high emotions, trauma, and grief of constituents with calmness and empathy to assess their current situation and take the next appropriate step(s) while upholding the Alzheimer's Association policy on client confidentiality.
  • Build rapport to engage constituents through various programs and services to help them better understand Alzheimer's disease, the progression, and the needs of people living with the disease.
  • Provide accurate and efficient documentation for interactions including the presenting issue and goal, interventions, and action plans.
  • Ability to plan and execute social engagement events and facilitate support groups within a territory.
  • Determine appropriate interventions and resources to link callers to education, support, local community resources, and emergency/crisis services if needed.
  • Follow established Association Helpline/Care Consultation policies, procedures, business rules, and protocols.
  • Achieves and implements grant funded projects (where applicable) to achieve Association objectives in accordance with enterprise grant policies and protocols.
  • Ensures an adequate number of active volunteers are recruited to meet program needs through a comprehensive recruitment plan that leverages technology, tools and staff across all functions
    • Provides technical assistance, mentorship and support to Program Volunteers ensuring volunteer onboarding and accountability by regularly monitoring volunteer impact and taking action to improve volunteer performance
    • Drives ongoing retention and volunteer relationship management, including adoption of new tools and technology

Qualifications

  • Bachelor's degree in human services, social work, psychology, counseling or equivalent experience in related fields.

Knowledge, Skills and Abilities

  • Proficient counseling skills in listening, assessment, empathy, collaboration, goal setting, and action planning.
  • Ability to handle interactions with constituents involving heightened emotions including crisis, abuse, suicide/homicide risk.
  • Attention to detail and effective time management and multi-tasking skills.
  • Familiarity with healthcare systems, mental health, aging and disability services, and Alzheimer's disease/dementia resources.
  • Proven experience in community service development and sustained program integration.
  • Physical Requirements: Prolonged periods of sitting at a desk and working on a computer. Travel, including evening and weekend work required.

Title: Care Consultant

Position Location: Syracuse, NY

Full time, based on 37.5 hours per week minimum

Position Grade: 205 ($58,500 annually)
