Family Medicine APP (NP or PA) in Sauk Centre, MN

CentraCare is hiring a full-time Family Medicine Advanced Practice Provider in Sauk Centre, Minnesota.  Our CentraCare - Sauk Centre clinic offers convenient access to care in a family-oriented clinic, focused on supporting patients wellness at any age.  This position is open to new graduates, able to see all ages from infants through geriatrics, working in a team-based approach.

Responsibilities

As a provider working in a team-based care setting, you will be paired with two physicians to provide care to patients with acute to chronic care needs.  Our primary care providers diagnose and treat acute short-term illness, provide preventative care including routine checkups, perform health-risk assessments, give immunizations and screening tests, offer personalized counseling on maintaining a healthy lifestyle, as well as manage and coordinate care for those with chronic conditions. 

Schedule

• Full-time
• Primarily Monday through Friday day shifts from 8a-5p, with one late shift per week 5p-7p
• In this position you would have the opportunity for Nursing Home rounds
• Saturday morning clinic rotation (approximately 13 per year)
• No holidays, call, or nights

Requirements

 To be eligible for this position you must meet the following requirements:

• Completion of a formal nurse practitioner or physician assistant program approved by a national accrediting agency
• Certified as a Nurse Practitioner or a Physician Assistant
• Current/pending licensure in the State of MN, licensed to see all age ranges infants and up
• BLS Certification

Experience

Position is open to new graduates with relevant work experience.  AGNP certification accepted.

Relevant work experience in one of the following areas helpful:

• Family Medicine
• Emergency Medicine
• Urgent Care/Same Day/Walk In
• Pediatrics
• Internal Medicine
