The stated pay scale reflects the range that Sharp reasonably expects to pay for this position.  The actual pay rate and pay grade for this position will be dependent on a variety of factors, including an applicant’s years of experience, unique skills and abilities, education, alignment with similar internal candidates, marketplace factors, other requirements for the position, and employer business practices. 


 


What You Will Do
The Senior Specialist - Hospice Pharmacy shall serve as a liaison between Sharp Centralized Pharmacy and Sharp HospiceCare to facilitate the development, implementation, and coordination of guidelines, pharmacy standardization of formularies, competencies and clinical practices as well as monitoring of control programs designed to minimize the incidence of medication events and enhance patient safety.

Required Qualifications

  • Education pursuant to licensure requirement.
  • AHA Basic Life Support for Healthcare Professional (AHA BLS Healthcare) - American Heart Association -REQUIRED
  • California Pharmacist License - CA State Board of Pharmacy -REQUIRED


Preferred Qualifications

  • 5 Years Experience in a hospice/palliative care environment.
  • 1 Year Experience utilizing Lean Sig Sigma or similar performance improvement methodologies.


Other Qualification Requirements

  • Current license to practice pharmacy in the state of California without restriction or probation.

    Identifies opportunities for quality improvements from surveillance activities. Serves as a facilitator to capitalize on them.
    Assists in developing and revising policies and procedures related to pharmacotherapeutics and related regulatory compliance.
    Continually works toward enhancement of the contribution and role of pharmacy in the improvement patient medication outcomes.
    Participates in medical staff advisory groups, multi-site design or multidisciplinary teams, focus groups, task forces, or other designated activities that lead to improvement in safe medication use and regulatory compliance.
    Assists with dashboard development and maintenance.
  • Problem resolution
    Utilizes methods and tools of performance improvement, to identify problems and initiate action plans to reduce the risk for MSEs.
    Provides consultative services/relevant educational programs on MSE/ADE control and prevention.
    Identifies opportunities for cost and quality improvements from the surveillance activities. Serves as a facilitator to capitalize on them.
    Identifies learning needs of staff. Presents and/or coordinates educational programs that lead to changes in knowledge and practice and may consist of educational in-services to the medical (including committee presentations), nursing or pharmacy staffs, or patients.
    Assists in developing and revising policies and procedures related to ADE control and prevention.
    As appropriate, represents Pharmacy in sentinel event evaluations, Root Cause Analysis (RCA) and Failure Mode Effects and Critically Analysis (FMECA).
  • Regulatory responsibilities
    Maintains thorough knowledge of current regulatory expectations affecting the provision of clinical pharmacy services and therapeutic medication usage in general, assisting customers to remain compliant.
  • Responsible for actions which result in improved patient and department outcomes
    Consistently demonstrates self-motivation in identifying opportunities for pharmacy to contribute more greatly to patient care by improving outcomes. Seeks out opportunities for unique pharmacy contribution toward enhancing the success of fellow care providers and patients.
    Continually works toward enhancement of the contribution and role of pharmacy in the reduction of ADE and improvement in patient care.
    Participates as appropriate in medical staff advisory groups, multi-site design or multidisciplinary teams, focus groups, task forces, or other designated activities that lead to improvement in safe medication use.
    Facilitates and keeps up with medication safety practices.


Knowledge, Skills, and Abilities

  • Proficient computer skills, especially in the application of word processing, spreadsheet, and database creation and management is preferred.
