Nutritionist for support of a WIC program; South Side of Chicago Our client, a network of five Federally Qualified Health Centers (FQHC) located on the south side of Chicago has an immediate staffing need for an experienced full-time Nutritionist. This is an on-site full-time position working M-F from (7:30am to 4:30pm). We understand that you cannot submit any comments or text with your resume. Job Responsibilities Responsible for nutritional needs of patients/provide dietary options w/i nutritional guidelines under a WIC program grant. Conduct Health Promotion/Disease Prevention (HP/DP) programs for patients/staff/community. Conduct short and long-term nutritional counseling for CFHC patients and their families. Provide nutritional consultation to CFHC professional staff. Schedule counseling sessions as needed per patient. Utilize EMR notes for communication w/referring providers re: progress of patients following each visit. Monitor individual progress by follow-up calls and mailings. Participate in case conferences with referring providers as needed. Conduct HP/DP programs on and off site, to CFHC patients & community residents. Evaluate all educational material for appropriateness. Write curriculum objectives, goals, and evaluations for each educational class and program. Coordinate outreach programs & promote nutritional awareness at CFHC, community agencies, and schools. Provide service training for CFHC staff on the nutritional aspect of the disease process, their prevention, and treatment with emphasis on comprehensive primary care and proper referral. Education & Job Requirements At least 1 year of recent and related Nutritionist or Dietician work experience. Must be a graduate of an accredited nutrition program w/BS in Food Science or Dietetics. Current licensure in the State of IL as a Registered Dietitian or significant progress toward obtaining license is encouraged. Someone who is fluently bilingual in (English/Spanish) read, write, and speak is preferred, but is not required.
