Community Health Centers of South Central Texas, Inc. (CHCSCT, Inc.), a FQHC (not for profit), is currently seeking a Family Nurse Practitioner/Physician Assistant for our Victoria Community Health Center location.
SUMMARY:
Provides health care services to patients under direction and responsibility of Medical Director by performing the following duties as determined by the credentialing and privileging of CHCSCT.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
- Evaluate and treat any patient presenting to any registered patient of the Community Health Centers of South Central Texas, Inc., following guidelines as set by the Medical Director.
- Obtains medical and developmental history and records findings in patient medical records.
- Performs complete history and physical examinations.
- Orders, interprets and reviews basic lab and radiographic studies (Xray mammogram) necessary for establishing diagnosis and the appropriate therapy.
- Promptly completes an encounter form for each patient visit.
- Recognizes common illnesses and injuries and consults with or refers to physician when necessary.
- Prescribes selected diets and physical treatment as appropriate.
- Provides follow-up care and health instruction through telephone communications and scheduled clinic appointments.
- Will provide a contractually set minimum number primary care encounters for the contract year.
- Instructs, educates, and counsels patients individually or in groups.
- Performs family planning physicals and tests of cure.
- May perform minor surgical procedures in clinic as determined by prior credentialing.
- Serve on appropriate health center committees to include Compliance Improvement/Risk Management, Providers, Pharmacy, and Medical Staff.
- Develops and/or reviews annually, Nurse Practitioner provider protocols.
