Job Description

Position Purpose: To assist in the administration of Freeman Houses Tier II Facility, with emphasis on the supervision of the Social Service Department.

Responsibilities & Functions:

Assist the Program Director in achieving social service project goals for clients in the facility, including performing initial screenings, evaluations, and physical and emotional services.

· Assist the Program Director with developing orientation and training material for Social Service staff.

· Back up for the Program Director, managing day-to-day services at the facility when he/she is not available as requested by the Executive Director.

· Represents the Program Director at City and State meetings.

· Conduct conference calls with the DHS Program Administrator and other DHS staff in the absence of the Program Director.

· Supervises 2 Case Managers (Case Manager #2 and Case Manager #3).

· Meets daily with the Program Director to report on ongoing progress with clients and their progress to transition into permanent housing.

· Update Program Director with weekly casework activity reports from Case Workers and Housing Specialists.

· Annotates in the logbook, all admissions and discharges for the facility as mandated by the Department of Homeless Services (DHS), and the City and State of New York regulations.

· Sign off on a weekly basis case records completed by case managers and housing specialists.

· Act as a liaison to the community-based services that assist in providing services with the transition to permanent housing for the clients.

· Performs intake and assessment of newly arrived Conditional and Eligible families referred from HERO (Housing Emergency Referral Office), Department of Homeless Services (DHS) within the 48 hour window as mandated.

· Complete an initial needs assessment and act upon critical needs appropriately.

· Gathers weekly casework activity reports submitted by Case Managers, and compiles a monthly casework activity report, submitting a report to the Program Director.

· Gathers information on Relocation Case Summary forms from case managers and stores in a logbook for review.

· Maintains a resident complaint/grievance log for review by the Program Director and Executive Director.

· Collects on a weekly basis, all daily school attendance sheets for monthly reports.

· Review daily incident reports, gathering responses, and filing them chronologically for review by the Program Director and Executive Director, when requested.

· Ensure that social service staff completes work on time.

· Orients clients to the shelter programs, requirements, rules and regulations, and consequences.

· Assure that specific DHS Program Goals are consistently being met.

· Provide information and referral services to clients as needed or as specified in the independent living plan.

· Conference non-compliant clients with the Program Director and Case Manager, as needed.

· Conduct case conferences with the Program Director, Case Managers, and families to ensure that they achieve goals on their Individual Living Plan, leading to permanent Housing.

· Ensure all records are available for review by the Program Director and Executive Director, as requested.

· Perform monthly reviews of all caseload records, reporting completed reviews to the Program Director.

· Bilingual English-Spanish a plus.
