Job Requirements: * FL RN (Registered Nurse) license * Education: Associates Degree in Nursing * Experience: Minimum of three years of Acute Care RN experience within a hospital setting required. * Minimum 2 Years of RN Case Management within a Large Acute Care "Hospital". * 1 year of Utilization Review experience required About the Job: * Working closely with the collaborative care team, the case manager assesses plans, intervenes, and reassesses an individual's needs to implement a plan of care to meet the needs of that individual * The case manager assesses patient and family for clinical appropriateness for the setting and establishes plans for effective allocation of resources and management of the continuum of care. * Utilization review is conducted according to ShorePoint Health guidelines * Coordinates the overall interdisciplinary plan of care for a patient, from admission to discharge * Acts as a liaison between patient/family and healthcare personnel to ensure necessary care is provided promptly and effectively. * Day Shift Available (8X5's varying days) INDBFH
