DIRECT SUPPORT PROFESSIONAL/CNA

Summary: Provide skills training, assistance, support and or supervision to individuals with disabilities in their home or a supportive living setting. Duties and Responsibilities: • Assist with developing daily living and socialization skills to include but not limited to, cooking, shopping, housecleaning, medication administration, personal hygiene and managing personal business. • Insure participation in valued activities based on the individual’s choice. • Responsible for implementing an approved behavior treatment plan including supportive counseling. • Should be able to communicate with Case Manager, Vocational Staff, Family members, and other community contact/resources. • Transport to and from appointment and other community resources. • Complete daily documentation and required program data as instructed. • Assist with completing assessments of individual skill training, assistance and support needs to develop goals and objectives for Individual Service Plan. • Perform other duties as assigned. Education/Experience Requirements: • High school education or equivalent, at least 18 years of age. • Six (6) months training and/or working with individuals with disabilities preferred but not required or any equivalent combination of training and experience that provides the required knowledge, skills and abilities needed to perform the required duties of the position. Required Knowledge/Characteristics of person with disabilities Skills and abilities: 1. Must have a current CPR/First aid 2. Good documentation and communication skills 3. Ability to implement ISP and collect data 4. Valid Driver’s license and acceptable driving record 5. Reliable transportation and vehicle insurance 6. Must demonstrate the ability to work independently with minimal supervision 7. Successfully complete behavior management training 8. Subject to criminal background check and child protective services central registry check 9. Must be flexible and willing to learn
