We are an organization focusing on the care and treatment of those suffering from substance and alcohol use disorder. Our patients are our focus and we always put quality of care above all else!
We are seeking Patient Care Coordinators to join our team. Patient Care Coordinator will be responsible for providing support to patients to reduce barriers to treatment and keeping patients engaged in addiction treatment services. They will model competency in recovery and ongoing coping skills while encouraging and inspiring hope for a patient in recovery.

Shifts needed are:

Overnight: 7:00pm to 7:00 am
Day: 7:00 am to 7:00 pm

20 minutes from Lockport

Essential Duties & Responsibilities:

Interact with patients in a professional, courteous, empathetic, and supportive way at all times.
Regularly communicate with Counselors and Nursing staff regarding patient issues and patient care.
Encourage positive interactions and conversations amongst patients and staff.
Transport patients throughout the facility.
Assure patients adhere to program activity schedule.
Conduct room checks on all patients.
Perform census checks every 2 hours (4 times per shift) including after meals.
Observe clients for inconsistent behavior and report to appropriate staff (Clinical, Nursing, etc.) and include this information in your shift report.
Assist in the de-escalation of behaviorally compromised patients.
Monitor patient linen and laundry.
Monitor linen supplies and replenish as needed.
Ensure soiled linen is placed in laundry bin at the end of every shift.
Collect laundry bags of dirty clothing and prepare for housekeeping to wash.
Deliver clean laundry back to patients.
Search and record new admissions belongings.
Assist staff with screening the client and their property for contraband.
Performs other duties as assigned by supervisor.
Act as a role model for patients and peers!

Minimum Qualifications:

High school diploma or equivalent
One (1) year of related work experience
