Job Description Summary:

Join the OhioHealth team as a home health nurse! Experience one on one patient care, a flexible schedule, and a great benefits package. Home health RN nurse case managers work with an interdisciplinary team to care for patients in the home setting. Join our amazing home health team where you have autonomy, a reasonable and fair weekend rotation, a flexible schedule, and limited on-call. Home health RN’s provide skilled care to patients to achieve outcomes in collaboration with the patient’s provider.

Minimum Qualifications:

Associate's Degree (Required)DL - Driver's License - Department of Motor Vehicles, RN - Registered Nurse - Ohio Board of Nursing

Demonstrates knowledge and skills related to the assessment and care of the Home Health patient. Valid State Driver's License - and maintains automobile liability insurance. Minimum one-year hospital nursing Experience . Preferred Previous Experience in home health. 3 years of hospital/home health Experience .
