POSITION SUMMARY: Under the supervision of integrated BH Manager provide the integrated behavioral health services/case management at AHS Family Health Center (AHSFHC).

DUTIES AND RESPONSIBILITIES:

  • Assessment of mental health needs, treatment planning, coordination and implementation of behavioral health services for patients.
  • Has thorough knowledge of current trends in local, state and federal programs in the behavioral healthcare field.
  • This position is a member of the multidisciplinary, primary care team and communicates and interacts with all levels of staff, primary care providers and health center patients.
  • Responsible for the adherence and maintenance of Mental Health policies and procedures for the behavioral health program.
  • Evaluates and assesses (provides intakes) for patients in a timely and courteous manner.
  • Performs counseling and crisis intervention as necessary.
  • Refers patients with serious mental health conditions necessitating inpatient or specialty care to appropriate institution or agency outside of AHSFHC.
  • Refer patients to MAT services.
  • Responsible to screen patients for prescription drug abuse, illicit drug abuse and alcohol abuse - Provide counseling to patients who screen positive or refer them to appropriate resources available in the service area.
  • Annual productivity should correspond to the minimum national average of a LCPC/LCSW in a FQHC setting or in accordance with the program expectations of the funding organization. Productivity expectations will be prorated accordingly for full time LCPC.
  • Maintains License by attending CEU trainings.

QUALIFICATIONS:

  • LCPC or LCSW with a Masters degree from an accredited school required.
  • Demonstrates excellent clinical skills
  • Evidence of mature judgment and crisis intervention skills.
  • Ability to effectively present information both orally and written, to administration.
  • Ability to work as a team member.
  • Computer literacy is required and ability to process patient records electronically.
  • Ability to work with a culturally diverse team.
  • Urdu/Hindi or other Asian language - preferred.
