JOB DESCRIPTION
Title: GPD Case Manager
Status: Full-time
Exemption Status: Non-exempt
Supervisor: Lead Case Manager / Program Manager

Job Summary:
The GPD Case Managers are responsible for the provision of case management and housing services to veteran homeless households assisted through the GPD Program. The Case Manager provides a variety of housing services to address housing barriers and to increase housing stability, including service coordination to ensure that participants are connected to the VA and other necessary supportive services. Qualifying Veteran applicants are given preference.

Responsibilities:
• Assist in screening all potential candidates and assess the potential candidate’s ability to live independently.
• Perform bio/psycho/ social assessments on Veteran family households to determine types of needs as they relate to housing stability.
• Actively involve clients in the design and delivery of supportive services by ensuring they have an active voice in their goal/service plans.
• Develop an individual self-sufficiency/service plan with each incoming household, with periodic changes as the needs of the participants change.
• Provide Program Support Services and Service Coordination including: Veteran Services Enrollment, Crisis Intervention, Rehab, Vocational, and Employment Assistance, general health and dental services, income support and benefits, Substance Abuse treatments, mental health services and participant and family involvement.
• Assist participants in gaining housing and maximize their independence and self-sufficiency by providing supportive services and appropriate community services and resources.
• Conduct regular meetings with participants to identify short and long-term goals, money management, and employment/education issues with housing as the first goal. Client meeting duration determined in coordination with supervising case manager or Program Manager.
• Conduct ongoing assessment of client needs, strengths, and resources in order to create plan and provide appropriate services.
• Develop collaborative relationships with the Veteran Administration providers and other local service agencies.
• Ensure that all children are enrolled in school and/or developmentally appropriate childcare as needed, assisting parents if necessary, with this process.
• Coordinate with schools, community non-profit agencies and other funding sources to provide tutoring, summer programs, and other resources for families.
• Maintain and secure comprehensive case files and prepare and submit all programmatic reports as required.
• Provide information and recommendations to the Program Manager/ Lead Case Manager regarding program evaluation and modification to better meet resident needs, community needs, funding requirements and Mission.
• Provide assistance to Case Manager Supervisor, Program Manager/ Director as assigned.
• Coordinate tasks and communication for participant with support staff

Qualifications:
• MSW, LCSW or Bachelor’s Degree from an accredited university or college in a related field, or comparable experience.
• Clinical/SUD Experience desired
• Minimum 2 years case management experience providing services to homeless or low-income individuals or families.
• Knowledge of and ability to use Harm Reduction and Motivational Interviewing techniques.
• Ability to work effectively and independently in community-based settings.
• Experience compiling and maintaining data records.
• Effective oral and written communication
• Excellent organization and time-management skills
• Initiative, flexibility, self-motivated, and capacity to respond effectively in stressful situations.
• Experience with community networking and resource building.
• Must have own transportation and a clean driving record.
• Willingness to be flexible with work schedule.
