RN, QAPI – Home Health

Here is an opportunity to join VNA Quality Assurance Program Integrity team as a QAPI RN for Home Health. The QAPI RN has a variety of responsibilities including development of process improvement plans, record reviews, data tracking and analysis, and serves as a regulatory resource for clinical staff. Position Summary: Manages assigned aspects of the QAPI Program for Home Health to include compliance with Medicare Conditions of Participation (CoPs), and regulatory, and accreditation requirements. Performs and uses audit results to lead development of Plan of Correction and Performance Improvement Plan. Prepares and presents audit results at Agency meetings.

Essential Functions:

Manages and performs required and requested audits from various sources.
Utilizing audit data, creates, implements, and monitors a plan of correction and performance improvement activities under the direction of the Director of Quality.
Manages the Process Improvement Plan (PIP) until process control is attained.
Collaborates with managers, healthcare disciplines, and internal staff to establish and uphold high quality standards of care.

Other Duties:

Participates in Agency’s QAPI activities, including but not limited to, patient incidents, employee incidents, infection tracking, and quality indicators such as OASIS (Outcome and Assessment Information Set) and patient satisfaction.
Analyzes corporate indicators data and other comparative data.
Continually assesses clinical staff compliance as required by internal policies and external standards of performance.
Provides follow-up clinician education and training in collaboration with the Clinical Educator.
Assists in development and implementation of policies and procedures which meet CMS (Centers for Medicare and Medicaid Services), accreditation, and federal, state, and local law requirements.
Performs clinical record audits and provides educational feedback to staff.
Assists/supports process for response to fiscal intermediary Additional Development Requests (ADRs).
Participates in quality projects including audit requests from payor sources.
Serves as a regulatory resource for the clinical teams.
Establishes and maintains highly effective working relationships with colleagues.
Performs other related duties as requested.

Knowledge and Experience:

Thorough knowledge of federal and state regulatory standards and accreditation requirements for Home Health.
Working knowledge of how a QAPI Program works and its purpose.
Keen analytic and critical thinking skills.
Understands and applies principles of adult learning when working with clinical staff.
Self-directed with ability to complete work/projects with minimal supervision.
Excellent verbal and written communication skills.
Highly organized with attention to detail.
Embraces the spirit of teamwork and displays a positive, can do mindset.

Education: RN with Bachelor of Science in Nursing or other related advanced degree.
