Home Health - Social Worker - Washington, MO

Full Time or Part Time Available

$5,000 sign on bonus!

• This position provides in-home social work services to patients within the following Mercy footprint.
• Servicing the following counties: Franklin County, Crawford County, Gasconade County, and Warren County
• Gas mileage reimbursement: $0.655/Mi
• Day One Medical Benefits, PTO Accrual, 401K Match

Overview:

To assist clinic patients to access services and referrals as needed. To follow clinic policies and procedures for all financial and adherence requirements.

Qualifications:

• Education: Master's in Social Work required.
• Experience: 0-2 years of Clinical Experience.
• Preferred Experience: One year of clinical social work.
• Other: Excellent interpersonal and communication skills. Knowledgeable in area of social services agencies, state regulations, and professional board standards as related to patient treatment, patient rights and client/patient confidentiality.
