Position Summary

The Salvation Army Northern Division Grand Rapids is hiring a Case Manager. The Case Manager - Rapid Rehousing (RRH) is responsible for providing a full range of case management services to RRH clients.

Essential Functions (approximate time spent)

• Provide strength-based, client-centered case management to RRH clients (60%).
• Maintain up-to-date, accurate client information in casefiles and through database entry (15%).
• Coordinate RRH recruitment, referrals, screenings, and intakes (10%).
• Engage in internal and external relationship building to better serve clients, develop new resources, and achieve program goals (10%).
• Maintain a current catalogue of local community housing resources and providers (5%).

Education and Experience

• Bachelor's degree in relevant field required.
• If degree is in social work, must be licensed by state board.
• Previous social services required.
• Experience conducting case management required.
• Must pass background checks, possess a valid driver's license, and meet MVR requirements.

• Proficiency with PC, Microsoft Office, databases, and standard office equipment.

Working Conditions

• Mostly sedentary work, sitting for long periods of time and working on a computer.
• Professional environment; eligible for a remote-friendly work environment.
• Must be able to communicate clearly in English, and work in person or on the phone.
• Must be able to occasionally lift 30 pounds.
• Standard business hours with occasional evenings and weekends.

Supervisory Responsibilities

None.

Travel Requirements

Will require frequent travel in Northeast Minnesota 6 county area and rare out-of-area travel.
