Reporting Structure: The Home Health Nurse Manager will report directly to the Nurse Director of Home Health. They will be supported by an experienced and tenured staff of Home Health Registered Nurses. 

Job Responsibilities for the Home Health Nurse Manager 

• Develop and implement care plans for patients receiving home health services 
• Supervise a team of home health nurses ensuring they provide high-quality care 
• Monitor and evaluate the quality of care provided and ensure adherence to care protocols. 
• Training and mentoring staff, and facilitating continuing education. 
• Ensure compliance with regulatory and accreditation standards 
• Assist in the development of home health policies and procedures to enhance the quality and safety of care
