An award winning medical center is searching for a an experienced Inpatient Case Manager to join their Care Management team in the gorgeous state of Oregon!

The Inpatient Case Manager will have the amazing opportunity to join a well established 25+ bed community hospital. This medical center has served as a valuable resource in its community and is located in the beautiful area of Central Oregon. This facility has been recognized as one of the top 100 Critical Access Hospitals in America. 

The Inpatient Case Manager will report to the Director of Case Management and will collaborate with Department Nurse Managers and Physicians. The Hospital Case Manager will play a vital role in ensuring that patients have a smooth transition from an inpatient acute care setting to a post acute rehabilitation program or skilled nursing facility. The Inpatient Case Manager will be responsible for organizing complex care, assessing patient's health insurance plans and providing necessary resources for patients being discharged. The Case Manager will act as an advocate for their patients and go above and beyond to help accurately assess their individual needs.
