*JOB DESCRIPTION:*
A LPN promotes the residents' health, wellness and illness prevention through daily nursing and personal care endeavors. A LPN works under the direction of a Registered Nurse and handles a diverse range of tasks, including administering treatment or medications, performing clinical procedures, and documenting the clinical data of patients, among other duties.
*RESPONSIBILITIES:*
* Ensures the care and safety of all residents in the nursing unit
* Ensures that individual care plans are followed
* Administers and documents medications, treatments, and other nursing services
* Makes a count of controlled substances on the assigned nursing unit
* Records current condition of assigned resident in EMR
* Receives and gives an accurate report of the resident condition at shift change
* Communicates to the nursing team and medical provider as indicated and required
* Assists in the orientation of new staff
* Maintains competency in all the tasks needed by CMS
*REQUIREMENTS:*
* Minimum 18 years of age
* New York State (LPN) Licensed Practical Nurse licensure.
* Strong written and verbal communication skills; able to follow written and verbal instructions.
* Basic computer knowledge, PCC e-MAR experience preferred.
* Physical endurance for frequent activity; ability to lift a minimum of 20 pounds.
