The Gardens at Scranton is seeking a Community Liaison for our skilled nursing facility in Scranton, PA.

 

The Community Liaison is responsible for generating business through qualified leads for assigned locations. Coordinate external activities that position the company as the leader in services and provider of choice in the market.

Responsibilities:

  • Clinical assessment of electronic medical charts from hospitals to determine if a facility can meet the clinical needs.
  • Verification of benefits
  • Obtain pre-authorizations from insurance company
  • Communicate clinical information to facility
  • Ensure closure with each referral.
  • Initiation of leads through relationships with qualified sources and contact with patients and families
  • Identify referral opportunities through market intelligence and territory management data
  •  Develop and implement special events and presentations to assist in maximizing fair market share
  • Collaborate with appropriate center employees and/or Call Center to determine new sources for leads

Qualifications:

  • RN/LPN/LSW or 1 / 2+ years' experience in nursing home industry in one of the following roles: Central Admissions, Director of Admissions, Clinical Liaison, Director of Marketing, and/or Social Services
  • Experience in healthcare related lead generation preferred
  • Experience in post acute or acute organization preferred
  • Ability to travel extensively
