GENERAL DESCRIPTION

The Clinical Social Worker will treat clients across the lifespan, with the full range of mental health conditions within the primary assignment area. The Worker provides mental health consultation, assessment, diagnosis, and treatment under the supervision of the Clinical Director/Executive Director as needed.

The Worker works with patients and families experiencing various psychiatric, medical, and social problems utilizing individual, group, and family counseling and therapy skills. The Worker will operate from a psychosocial rehabilitation and recovery model of care during all client and family interactions.

Duties may be assigned in other program areas as needed for the overall benefit of this integrated health organization. Further, the Worker will provide a full range of therapeutic interventions including psychosocial assessment, individual therapy, group therapy, and psycho-educational classes. Best practices, practice guidelines, and evidence-based treatment protocols are utilized to the extent feasible in all therapeutic interventions.

1. FUNCTIONS OF POSITION

Independently performs case-finding functions and outreach, identifying and screening patients for psychosocial needs. The Worker must possess the knowledge and ability to independently implement treatment modalities, and provide counseling or psychotherapy for individuals, families, and groups. Psychotherapy will be evidence-based in nature whenever available for a particular condition, and the Worker will utilize best practices to the extent possible in all patient-care interactions.

Conducts biopsychosocial assessments under the supervision of a qualified Licensed Clinical Social Worker.

Utilizes the most recent edition of the Diagnostic and Statistical Manual of Mental Disorders DSM) as the basis for diagnostic impressions and conclusions.

Develops person-centered, culturally sensitive treatment plans in collaboration with the client/family to address the mental health needs of patients.

 

 

Provides psychosocial interventions and measures outcomes of interventions.

 

Comply with national and local performance mandates, including timely healthcare

screening and completion of applicable clinical reminders for all patients.

 

Coordinates community-based services, including information and referral for additional services from other programs and community agency programs.

Identifies high-risk patients and provides case management services as needed.

Provides crisis intervention services, seeking to address the cause as well as the presenting complaint, coordinates family conferences, and serves as liaison to family members.

Establishes and maintains effective therapeutic relationships with clients and their families. The Worker can independently work with patients and families who are experiencing a variety of psychiatric, medical, and social problems utilizing individual, group, and family counseling and therapy skills. The Worker will operate from a psychosocial rehabilitation and recovery model of care during all client, patient, and family interactions

Communicates effectively, both orally and in writing, with people from diverse educational, socioeconomic, cultural, and ethnic backgrounds

Completes assigned Performance Improvement activities at the program, service line, and medical center levels.

Demonstrates general knowledge of resources available to the client and facilitates referrals based on the client’s needs and eligibility.

2. QUALIFICATIONS

Must have a working knowledge and experience in the use of medical and mental health diagnoses, disabilities, and treatment procedures this includes acute, chronic, and traumatic illnesses/injuries, common medications and their effects/side effects, and medical terminology.

Must be able to provide the client with consistent information according to established policies and procedures

 

 

The Clinical Social Worker must have a master’s degree in social work granted by a graduate program fully accredited. Must be an LCSW or LCSW-A licensed in NC.

Has working knowledge and skill in the use of word processing, data management, and other computer systems, especially those programs in use by Southeastern Healthcare.

Must have a valid, unrestricted North Carolina driving license if the position requires the operation of a COMPANY vehicle in the performance of assigned duties.
