Certified Home Health Aide (CHHA)

QUALIFICATIONS:

* High school graduate or GDE

* Ability to read and write consistence with job requirements

* Completion of a Home Health Aide training program consistent with State and Federal requirements and for which a Certificate was obtained

* Satisfactory performance on a competency evaluation and annual re-evaluation

* One year experience of employment as a Home Health Aide

* Must have a reliable means of transportation

RESPONSIBILITIES:

* Participate in the planning, coordination and implementation of the PLAN OF CARE

* Task to be performed by HHA will be assigned and performed under the supervision of RN

* Helps the client to maintain good personal hygiene

* Assist in maintaining a healthful , safe environment

* Plans and prepare nutritious meals

* Assist with certain treatment as ordered by the physician and approved by the nurse
