Responsibilities

Evaluates (assesses), plans, implements, coordinates, and monitors the options and services to help fulfill the person’s health needs, using the available resources to promote quality and cost-effective results. Advocates for the consumer and serves as liaison with the community and governmental agencies and with the Independent Practitioner Associations (IPA). Establishes the treatment plan with consumer and discuss it in the ICT meeting. Takes into consideration the consumer’s strengths to develop the treatment plan. Involves the family and significant members of the community in the treatment with the consumer. Makes sure that the patient has access to all the available tools for his or her rehabilitation and for a better quality of life.

1.     Informs the member about the services provided by the Case Management Program. Facilitates informed options and decision-making. Acquaints the member with the consent to receive program services, to disclose and obtain information, the complaint and grievance process, and with any other related information

2.     Promotes the established interdepartmental communications systems with the Quality, Financial, Pharmacy, Utilization, Access Center departments, among others to coordinate services and track how our member make use of the mental health services.

3.     Promotes the recovery and well-being of the insured incorporating in the provision of case management services practices that are supported by the Recovery Model

4.     Post-discharge follow-up phone calls after psychiatric hospitalizations

5.     Conduct the interview for the intake assessment, PHQ9, suicide risk assessment, and other screening log.

6.     Complies and promotes the established confidentiality regulations

7.     Documents the clinical information in the record or in the appropriate platform and logs member’s file.

8.     Prepares monthly reports establish by the organization based in our client’s needs.

9.     Visits the consumer at home or at another accessible place

10.  Coordinates and visits the community agencies or services including IPA’s of the integrated system in accordance with the consumer’s needs

11.  Performs case discussions with the physicians or with the program or clinic administration

12.  Participates in case discussions with mental health providers and other community or governmental agencies professionals

13.  Establishes a clear and concise consumer continued treatment plan with the member.

14.  Monitors the recovery plan, measures progress, and establishes the discharge date or continuation of services if necessary

15.  Advises the person, the family, and significant others of the options available for their recovery and self-management tools.

16.  Fills in the encounter sheet for each intervention carried out

17.  In compliance with the policies, procedures, and code of ethics maintains a therapeutic relationship with the family or support system

18.  Discuss service as a liaison between member’s family, mental health providers and physical health providers.

19.  Practices cultural competency, conscientiously, and with respect for diversity

20.  Implements models of collaborative practice to include therapists in the mental health field and the primary doctor, including the four quadrant model.

21.  Promotes and incorporates the use of evidence-based practices and empowerment strategies with an emphasis on prevention of exacerbations and complications

22.  Maintains an adequate case load

23.  Represents the program or the corporation in interdepartmental meetings, with government agencies, commercial accounts, IPA’s, and profit and nonprofit organizations, among others

24.  Keeps the specialization license and professional association membership (colegiación) effective

25.  Complies with the continuing education hours that corresponds to your specialization

26.  Participates in the mandatory trainings, such as: the Annual Compliance Plan, URAC, NCQA and CMS guidelines and regulations.

27.  Informs the supervisor or human resources of any sanction or revocation of your license that affects the practice of your profession

28.  Available to travel throughout Puerto Rico

29.  Other tasks assigned by the supervisor

30.  Participates in the ICT meetings.

Qualifications

  • Education: Masters in Social Work, Psychology, Psychological Counseling, Mental Health Counseling, or Psychiatric Nursing
  • Licenses: Valid and current active license for Social Work, Psychiatric Nursing, Psychological Counseling, Psychology, or Mental Health Counseling
  • Professional associating membership (colegiación): Valid membership for Social Work and Psychiatric Nursing
  • Relevant Work Experience: Two or more years of experience in the health field
  • Knowledge in Microsoft Office
  • Fully Bilingual
