Caring Hearts Home Care is looking for an experienced CNA, HHA or Caregiver who is ready to take that next step in their career! 

 

We are currently conducting interviews for a Care Coordinator to join our Caring Hearts Home Care family.

This individual must be motivated, possess a Caring Heart, and be passionate about providing exceptional service to our clients and their families.

This person will work closely with caregivers and clients, as well as our current office staff.  We are looking for the ultimate team player! 

 

Job responsibilities:

  • Assist Scheduling Coordinator to help match caregivers with clients
  • Ability to find potential caregivers and conduct new hire interviews
  • Assist with training, orientation and development of caregivers
  • Manage/Update caregiver and client software program personnel files
  • General office duties: filing, emailing, creating documents, etc.
  • Answer office phones; includes "on-call" phone after normal business hours (rotating basis)
  • Develop/Update client care plans; review with caregivers
  • Communicate with clients, caregivers and family members to ensure quality care
  • May be called upon to work on weekends (as needed)
  • Fill-in for caregivers until shift is filled (if necessary)
