Serenity Hospice Care has an immediate need for a Licensed Social Worker.
Serenity Hospice Care Social Workers perform the social services section of the assessment process; including but not limited to, completing the psych-social assessment, educating the patient and family about the Hospice benefit and gathering financial information.
Develops a plan of care with the interdisciplinary team, the patient and the family to deal with personal, financial and environmental difficulties experienced by the patient.
Assists the Interdisciplinary team to understand the significance of social, emotional and financial factors related to the patient's care.
Social Worker Assesses and reassesses social, emotional and financial factors in order to help the patient and family cope with problems related to the patient's lie limiting illness.
Identifies and utilizes community and family resources to assist with the patients plan of care. Develops, prepares and maintains clinical documentation with accuracy, timelines and according to prescribed policies.
Must be an LSW in the state of NJ.
One year hospice experience preferred but not required
Willing to travel
Be familiar with the territory
Social Worker Must work independently but also work well within an Interdisciplinary team.
