Vanderman Place -

Vanderman Place, a Skilled Nursing Facility, is currently seeking a Care Plan Coordinator. This is a 24 hour position, but there is full time availability in other roles, and will also be put into the on call rotation.

Job responsibilities include but are not limited to:

Maintain current interdisciplinary Care plans for all residents in the facility in accordance with all federal and state certification guidelines. This includes gathering information, assessing needs, establishing reasonable goals, providing interventions and incorporating them within an organized, concise, functional care plan.

Conducts interdisciplinary care conference meetings on admission, quarterly and as needed. Also responsible for inviting family members/ responsible parties, as well as the patient, to these meetings in writing.

Must have effective communication skills to be able to develop a rapport with families, co-workers, residents and families.

Develops and maintains patients CNA care cards on admission, quarterly, and as needed.

Ensures that flow sheets, CNA card cards, and treatment records are reflective of plan of care with follow through with the Nursing Supervisor.

Performs any work assignments that may be assigned.

Attends weekly Standards meeting and follows through on this assignment throughout the week.

Assist in the development of solutions for improvement. Directs in a collaborative manner with other Clinical Departments in the identification, investigation in implementation of continuous improvement opportunities directly related to professional/patient care practices and patient outcomes including Quality Assurance projects and nursing audits as needed or when directed.

Utilizes current quality improvement/problem solving knowledge and skills in day-to-day operations and strategic planning.

Must possess creativity, integrity, and initiative. Must be highly organized and computer proficient.

Acceptance of and ability to carry out responsibilities assigned.

General Requirements
• Graduate of an accredited nursing program

• Current nursing license, in good standing with the State of CT licensing board

• Minimum of one (1) year LTC experience require

• Previous MDS or care planning experience is preferred.

• Federal, State, and required credentials current and on file

• Cognitive skills as related to the position
