About this job

Adventist Health Care seeks to hire an experienced Care Navigator, Inpatient RN, who will embrace our mission to extend God's care through the ministry of physical, mental and spiritual healing.

The Inpatient Care Navigator collaborates with the patient, physician, case manager, and other healthcare providers to support patients as they move throughout the healthcare system.

As a Care Navigator you will:
•Identify, assess, plan, implement, coordinate, monitor, and evaluate healthcare services to meet an individual patient's specific health care needs in a cost-effective manner. (may include but not limited to home health assessment, telehealth initiation, disease management education, coordinate of post-acute services)
•Conduct patient home assessment visit when indicated
•Ensure weekly contact with the patient (in person or via telephone) to progress towards care plan goals. Conduct patient home assessment visit when indicated

Qualified candidates will possess:
• Graduate of an accredited school of nursing with a minimum of a Bachelor's Degree.
• Active Maryland RN license required
• Active American Heart Association Basic Life Support (BLS) certification required
• At least 5 years of recent nursing experience required, case management experience is a plus.
• 1-3 years experience in patient navigation, discharge planning, or care coordination
