Licensed Practical Nurse/LPN - Part Time, Nights Shift, Greenfield, WI



Community Care is hiring for a Licensed Practical Nurses/LPNs role who are committed to teamwork and providing great care to become a part of our compassionate caregiving team. The Licensed Practical Nurse/LPN works to staff to ensure the health, safety and welfare of program members. This is a part time opportunity on our night shift for Our Lady of Angels location at 3995 South 92nd Street, Greenfield, WI 53228.



Licensed Practical Nurse/LPN Job Responsibilities:



  • Work in collaboration?with other nursing staff to coordinate and provide direction of care during weekday operations.



  • Serves as coordinator of care for scheduled weekend and holiday shifts.



  • Ensures care and service delivery are in conformance with regulations, policies, procedures, and standards, established by Community Care, the Program for all-inclusive Care for the Elderly and the State of Wisconsin.


Licensed Practical Nurse/LPN Job Requirements:



  • Education:?Diploma from accredited program in practical nursing required.



  • Licensure: A current Wisconsin Licensed Practical Nurse/LPN license in good standing
    Experience: Minimum of 2 years nursing experience with a geriatric population required. Working knowledge of normative, developmental tasks of aging, knowledge or regulations governing site operations.



  • Certification: Current CPR/AED certification


Licensed Practical Nurse/LPN - Team Supervisor Job Schedule:



  • 10:30 PM – 7:00 AM 20 hours per week



  • Every other weekend



  • Every other holiday
