Our committed team of Licensed Vocational Nurse, Registered Nurses and Home Health Aides specialize in providing support to seniors, and adults and children who are dependent on gtube, trach and vents.

By providing continuous nursing care to medically fragile patients that qualify for skilled nursing shift care. At Chelsea’s Home Health Care, our nurses spend quality 1:1 time building a relationship with each of their patients. • Required care: Patient is a 4 year old male. Training and shadowing is provided. Care consist of monitoring patient status, maintaining activities of daily living, administrating medications, suctioning and breathing treatment and feeding via pump. All care is supervised by an RN case manager.

Credentials required:

• Current Licensed Vocational Nurse (LPN) License
• Current Physical
• Current PPD or Chest X-Ray.
• Current BLS card.
