Job Description
The project prevention coordinator focuses on the goal, of enrolling a total of 750 unduplicated male and female youth and young adults, ranging from ages 13-24. This initiative will engage the population of focus in prevention services toward a reduction in the onset of substance use/abuse, co-occurring mental health disorders, HIV/AIDS and viral hepatitis within minority communities of color.
Duties and Responsibilities
· Responsible for coordination of daily prevention activities including, but not limited to staff HIV/AIDS training, hepatitis and HIV counseling testing and referral training; group schedules, recruitment, testing, planning, implementing and evaluating client care through the use of best practice standards, as well as, ensuring all objectives are met
· Responsible for recruiting and building stakeholder relationships (e.g., schools, prison's, jails, probation officers, county health departments, community agencies, hospitals, etc.)
· Ensures all program participants complete the GPRA tool at intake, exit and follow-up
· Ensures linkage to care is provided for individuals testing positive or those who are HIV positive at admissions including those needing substance abuse treatment or co-occurring treatment and recovery support services
· Collaborate with community stakeholders for the purpose of planning and facilitating recruiting and marketing initiatives
· Adheres to project timelines and responsible for the development of programmatic report, which includes performance data on a monthly basis
· Works closely with the project's evaluator ensuring all reports are completed and submitted in a timely manner
· Identify, train staff and facilitate appropriate interventions for the population of focus.
· Responsible for ordering and managing test kits and all other supplies
· Embrace CARF A.S.P.I.R.E. to Excellence standards
· Participates in regular staff meetings, staff training, and accept the responsibility for aiding the development of positive team relationships
· Adhere to agency policies and procedures, as well as, the professional code of ethics
Qualifications and Education Requirements
· Georgia Certified Alcohol and Drug Counselor Level III (GCADC III), Certified Alcohol and Drug Counselor (CADC), Georgia Certified Alcohol and Drug Counselor II (GCADC II), Certified Addiction Counselor, Level II (CAC-II); Master’s Degree and licensed as a Licensed Clinical Social Worker (LCSW), Licensed Professional Counselor (LPC) or Licensed Marriage and Family Therapist (LMFT);
· Experience with HIV/AIDS and Hepatitis counseling, testing and referral.
· Ability to work with youth and young adults and ability to define problems, collect data, establish facts, and draw valid conclusion
