COMPANION HOSPICE HAS AN OPENING FOR 1 TELE-TRIAGE RN FOR THE FOLLOWING SHIFT(S):

1. Friday and Sunday - 4 PM - 12 AM, Saturday 8 AM - 5 PM

WORK FROM HOME to cover the Southern California service areas including the following counties; Los Angeles, Orange, Ventura, San Bernardino and Riverside.

ESSENTIAL DUTIES:  

The Tele-Triage Nurse will be responsible for patient triage, assist with nurse case management, nursing assessments, collaboration with providers and other departments, and provision of quality patient care in compliance with local, state and federal regulations and accreditation standards. The professional triage nurse builds from the resources of the organization to plan and direct services to meet the needs of the individual and families within their homes and communities based on the individualized Plan of Care. 

JOB REQUIREMENTS:

• Minimum of three years experience in direct patient care, preferably in hospice and/or medical surgical nursing care settings.
• Possession of current RN license  in the state of California, BSN preferred
• Self-directed and able to work with minimal supervision.
• Excellent verbal and written communication skills, including strong organizational, detail and interpersonal skills
• Bilingual Skills Preferred 
• Experience in multi-group health center setting preferred
