- Responsible for the coordination of health care decisions by using a systematic approach to ensure treatment plans that improve quality and outcomes
- Responsible for the promotion of cost-effective care within allotted time frame
- Assure payments of hospital-based services meet patient-related utilization of management criteria
- Implementation of safe and appropriate discharge plans
- Asses the social work needs of the patient and provide intervention as part of the discharge planning process
- Primary functions include: Clinical Interventions/Discharge, Care Planning Management, Fiscal Manage and Payer/Referral Management
JOB REQUIREMENTS FOR THE LICSW LICENSED CLINICAL SOCIAL WORKER - CASE MANAGER:
- Master's Degree in Social Work
- Current state licensure as a Licensed Social Worker - MUST BE LICSW
- Previous experience in case management or related area, hospital strongly preferred
- Must be eligible to work in the United States
