Performs household services that are essential to the Home Health patients care and are necessary to prevent or postpone institutionalization. 1.1 Helps the patient with personal care, i.e. bed bath, assisting with tub bath, etc. 1.2 When appropriate, assist in meal preparation and feeding. 1.3 Assists patient with dressing. 1.4 Helps the patient in and out of bed, wheelchair, etc. 1.5 Performs light housekeeping tasks, i.e. laundering the patient's gown or pajamas and cleaning the patient's room. Assists with certain treatments as ordered by the physician and approved and supervised by the appropriate personnel. 2.1 Performs simple dressing changes 2.2 Gives patient range of motion exercises. 2.3 Takes vital signs every visit. Assist with the maintenance of clinical notes. Exports clinical notes every working day by midnight. 3.1 Observes patient closely for signs and symptoms of change of behavior, condition of skin, color of skin, appetite, etc. 3.2 Reports any unusual condition or change in patient. 3.3 Reports all problems to the Case Manager or Supervisor. 3.4 Completes clinical notes in a timely manner in accordance with department standards. Assumes responsibility for meeting standards of Nurse Aide. 4.1 Attends conferences, inservice education classes and other department activities. 4.2 Keeps informed and continually strives to improve her knowledge of skills. 4.3 Maintains professional appearance, attitude and performance. 4.4 Knows policies and procedures of Home Health Department regarding nurse aide practice, safety, health hazards, and personnel policies. Demonstrates knowledge of special needs and behaviors of specific age groups including, but not limited to, adolescent, adult and geriatric. 6.1 Demonstrates knowledge of the changes associated with aging . 6.2 Demonstrates ability to provide care based upon physical, psychosocial, cultural, spiritual and other related factors. Performs other duties as assigned by the Supervisors, Managers. Because you're committed to excellence, you understand the importance of being properly prepared for your role at Mercy. That's why you'll bring to your role the right set of qualifications: Education: Must have a high school diploma or GED. Complete appropriate hours and pass test required for the position. Experience: Six months experience working with people and an interest in working with people. Certifications: Nursing Assistant or Home Health Aide certification. American Heart Association Basic Life Support (BLS) Required. Other: Preferred Experience: 2 years of experience in Home Health setting.
