Services Provided

Because We Care II provides Mental Health Support Services (MHSS), which are time-limited training and supports that assist consumers with achieving and maintaining community stability through teaching and reinforcement of independent living skills, identifying and using other resources in the community, assistance with medication management, and monitoring of physical health and nutrition. Individuals must demonstrate a clinical need for the service arising from a condition due to mental, behavioral, or emotional illness that results in significant functional impairments in major life activities. Individuals must meet at least two of the following criteria on a continuing or intermittent basis:

· Have difficulty in establishing or maintaining normal interpersonal relationships to such a degree that they are at risk of hospitalization, homelessness, or isolation from social supports.

· Exhibit such inappropriate behavior that repeated interventions by the mental health, social services, or judicial system are necessary.

· Exhibit difficulty in cognitive ability such that they are unable to recognize personal danger or recognize significantly inappropriate social behavior.

· Require help in basic living skills, such as maintaining personal hygiene, preparing food and maintaining adequate nutrition, or managing finances to such a degree that health or safety is jeopardized.

 

Role and Responsibilities

· Face to face meetings with the member and/or the parent/guardian/family member for the implementation of activities delineated in the individual plan of care

· Provide the adult consumer/supports with a comprehensive continuum of care by planning, coordinating, integrating and monitoring services

· Continuously determine needs of adults by assessing and reassessing status

· Participate in the development, review and revision of the Individualized Service Plans with adult recipients

· Provide support services designed to meet the educational, vocational, residential, financial, social, medical, mental health and/or substance treatment

· Serve as liaison between the consumer, mental health professionals, other service providers and agencies

· Complete all documentation, including progress notes and ISP according to Medicaid guidelines

· Attend required supervisions with assigned Licensed Professionals, per his/her supervision plan, by a review of progress notes, the consumer’s progress toward achieving ISP goals and objectives

· Work with team members, maintaining effective inter- and intra-agency working relationships

· Work independently, performing position duties under general supervision; and

· Promoting the enhancement of self-care, personal hygiene, selection of nutritional food, and appropriate eating habits
