Hospice After Hours 

 Monday-Thursday 5 pm-8 am

On Call Position expected to be available for patient triage and patient home visit if needed. Service area will include Round Rock to Temple.

The After Hours RN will function as part of an after-hours nursing team. The After Hours Registered Nurse will be responsible for following the written plan of care for each Hospice patient, family and caregiver prior to providing care and services as written and implemented by the RN Case Manager. RN will act as Team Leader on-call to resolve complex patient issues after-hours and partner with the Hospice Manager when needed. After Hours Registered Nurse provides nursing care to include the assessment, education, counseling and treatment of patients and delegating care to the others in the interdisciplinary team as appropriate.  The After Hours Registered Nurse will function under the supervision of the Hospice Manager.

 

Duties and Responsibilities:

Triage after hours calls from Hospice patients, make visits for symptom management

Resolve complex patient complaints, refer to Administrator on call as needed

Admit Hospice appropriate patients after hours, ordering medications and DME to mitigate patient pain and maintain comfort of the dying patient

Conduct comprehensive nursing assessment of the hospice patient at home, skilled nursing facility or anywhere patient calls home

Communicate with the Hospice Medical Director to resolve complex symptom management issues of the Hospice home patient

Determine General In-patient (GIP) eligibility for hospitalized patients for GIP admission to hospice service

Responsible for admitting, discharging, transferring, arranging for alternate levels of care, and attending the deaths of the Hospice patients

Demonstrates critical communication skills including self-awareness, inquiry/dialogue, conflict management, negotiation, advocacy and listening

Recognizes that resources may be limited and consider factors related to safety, effectiveness and efficiency in planning in delivering patient care

Uses strategies and available resources for problem solving for patient/family and staff

Creates a compassionate, supportive, safe and therapeutic environment for patients and families at the end of life

Utilize the nursing process, critical thinking to resolve complex patient concerns

Recognizes, appreciates, and incorporates unique differences of patient/family into an individualized plan of care

Functions as a team member and works collaboratively and independently, involving intra and inter-disciplinary work with colleague and community

Graduate of an accredited Program required

Registered Nurse (RN) license required

Basic Life Support (BLS) certification within 30 days of hire required

2+ Years of experience required


Note: If you have less than 6 months of experience, we recommend that you apply to our nursing internship program.  Internships are available for new graduates and experienced nurses in a variety of specialties. New classes begin in January, June, and September.
