Heartland Hospice, a leading provider of hospice care, is currently recruiting for a Care Transition Registered Nurse. We are seeking an experienced hospice, home health, disease management, case management, or oncology nurse to fill the role. This is primarily a Monday – Friday position. You will be educating the community in the Medical District of Chicagoland, through Oak Park and additional areas throughout Chicago. Bilingual in Spanish and English preferred.
The Care Transition RN/Nurse Liaison is responsible for providing support to transition a patient smoothly between care settings / levels of care. They work collaboratively with the patient, family, facility discharge planning team and Heartland team for the care transition of patients choosing hospice services. The responsibilities include explaining the benefits of hospice, assessing eligibility, and completing sign on to the Hospice benefit. The RN Liaison also partners with the Account Liaisons to educate, develop and maintain referral source relationships.

Educational Requirements
Registered Nurse required. Licensed and in good standing in the state in which he/she will practice.
Position Requirements
Minimum of two years of experience delivering home health care, IV care or hospice services.
Demonstrated knowledge of payer sources.
Demonstrated knowledge of the referral source types and community resources.
