Currently seeking a Per Diem Physical Therapist for a Beautiful Senior Living Community in Stockton, CA!

Therapy Specialists has a wonderful Per Diem Physical Therapist opportunity at a beautiful resort-style Senior Living Community in Stockton, CA. O'Connor Woods provides a caring environment for Skilled Nursing, as well as other long-term and short-term care needs. The Skilled Nursing Facility at O'Connor Woods is a 100-bed facility located on a lush and expansive campus in the heart of Central Valley. Our multi-disciplinary rehabilitation team also provides Outpatient Services to residents of the independent living and assisted living campus. The team at O'Connor Woods is a cohesive group of therapists that collaborate well together to provide evidence-based patient centered care aimed to achieve the individual's maximum functional level so that they may enjoy life's activities that are most meaningful to them!

We are looking for a Per Diem Physical Therapist with weekday and weekend availability to join our team. Additional assignments may be available at our sister facilities in Sacramento, CA and Turlock, CA.

Position Requirements:

  • An active Physical Therapist License in California
  • A minimum of 1 year of recent Skilled Nursing or Senior Living experience is preferred, but New Graduates with applicable clinical experience are welcome to apply
  • Ability to float from Skilled Nursing to provide outpatient and in-home services to residents on the campus in assisted and independent living
  • Casamba EMR experience is preferred
  • PDPM knowledge and experience working with Medicare Part B patients is preferred
  • Ability to train and supervise 1-2 PTAs at a given time

Position Responsibilities include, but are not limited to:

  • Screening, evaluating, and providing treatment in skilled nursing, outpatient, home health, and assisted & independent living
  • Providing caregiver and family training, as needed
  • Communicating with other disciplines nursing and staff, as needed
  • Completing thorough documentation and managing paperwork efficiently with independence
  • Supervising PTAs and effectively communicating with PTAs and the interdisciplinary team regarding goals and outcomes based on plan of care
  • Providing excellent customer service and patient care
