The RN Navigator enhances the patient experience by interfacing with various patient care teams and assists to provide comprehensive care to patients
who are in need of diagnostic and therapeutic procedures, and those newly diagnosed with medical conditions requiring treatment/follow-up.
The RN Navigator works collaboratively with radiologists (both interventional and diagnostic), genetic counselors, referring physicians, treating physicians, surgeons and other treatment navigators to ensure that the patient gets seamless care for optimum outcomes.
This position provides clinical functions as required in support of services and participates in maintaining a safe, cost-effective patient care environment. The RN Navigator also serves as a community liaison for education and awareness.
Responsibilities- Coordinates care for patients and families.
- Provides initial patient contact.
- Develops plan to assist patients with scheduling.
- Identifies patient trends, receives referrals from patients, coordinates housing for patients.
Qualifications:
Education
Graduate of an accredited Nursing program.
Preferred: BSN
Experience1 - 2 years experience in a clinical setting;
Preferred: 3 to 5 years in a RN Navigator role
Certifications/Licensure: RN License in the State of Arizona, or other Compact State, Basic Life Support; Preferred: RN Navigator CertificationJob:
