Overview

The Memory Care Director will plan, organize, develop and direct the overall operation of the Memory Care Unit in accordance with state regulations that govern the facility, and as directed by the Resident Care Director, Administrator and/or corporate structure to ensure the highest degree of quality care is maintained at all times. This will include, but not necessarily be limited to, assuring that care plans, assessments and other required paperwork is completed in a timely fashion; all required documentation is included in the residents record in a timely fashion and assuring that appropriate activities are provided to the residents on a regular basis.

Responsibilities

Supervises and carries out all functions and activities on the Memory Care Unit.

Manages resident record documentation in accordance with regulatory requirements.

Perform all administrative duties as required by the Administrator such as completing forms, reports, assessments, evaluations, charting, etc., as necessary in a timely fashion.
Coordinates all Family Meetings. This will include family conference meetings related to care issues. Coordination will include working with the Resident Care Director and Administrator to establish time frames for meetings and appropriate correspondence with families to assure maximum participation.
Assist with assessment of potential residents for admission in collaboration with Administrator / Resident Care Director.

Assure all employees complete necessary trainings as required by state regulations.
Responsible to assure appropriate quantities of supplies related to personal care.
Responsible for assuring that discharged resident records are properly "closed" and stored. This will include assuring that a discharge note is written and that all required documentation is accounted for, properly organized and archived in the appropriated manner.
Must be able to serve as Personal Care Aide and Medication Technician as necessary to assure proper staffing and proper care of the residents.
In conjunction with the Administrator/ Resident Care Director will hire and assure proper training of all staff on the Special Care Unit.
In conjunction with the Administrator and Resident Care Director, will provide on-going supervision that may include disciplinary measures as set forth by corporate policies and guidelines.
Will provide performance evaluations to employees as scheduled by corporate policies and guidelines.
Develop work schedules and assure that proper staffing patterns are always adhered to.
Assure duty assignments are developed each shift, each day; and/or assist supervisory staff in assuring these functions are complete.
Make daily rounds of the Special Care Unit personal care department to assure that all personnel are performing their work assignments in accordance with acceptable standards.
Perform any other duties as assigned by the Administrator and/or Resident Care Director to assure the proper care of the residents and effective functioning of the adult care facility.

Qualifications

  • At least 21 years of age.
  • High school diploma or GED equivalent
  • 3 years experience as a supervisor, working with the elderly, preferably with those that have dementia diagnosis.
  • Possess leadership and supervisory ability; willingness to function as an integral part of a team that includes professional, para-professional and non-professional personnel.
  • Possess the ability to work cooperatively with personnel, residents, family members, visitors, regulatory agencies and the general public.
  • Provide direction in a fast paced, stressful environment
  • Possess the ability to plan, organize, develop, implement and interpret the programs goals, objectives, policies and procedures that are necessary to assure quality care.
  • Be an independent thinker with the ability to make decisions that benefit the resident, staff and the organization as a whole.
  • Possess the ability to relate resident's conditions to the appropriate individuals (families, doctors, health care personnel etc...) to assure that the highest quality care is maintained.
  • Possess the ability to readily assess behaviors that may pose a threat to the resident, other residents or to employees, if not addressed in a timely fashion. Assure that proper follow through is provided to assure the residents safety.
  • Possess excellent documentation skills as well as the ability to review records on a regular basis to assure that all other required documentation is being completed in a timely and quality fashion.
  • Possess well-developed organizational skills
  • Strong commitment to working with individuals that have dementia diagnosis.
  • Be able to represent the facility in a professional and positive manner at all times, particularly when interacting with individuals not directly associated with the facility (vendors, family members, regulatory agencies etc...)
