Pediatric Pharmacist

Job Responsibilities

Pediatric Pharmacists ensure safe, optimal, and efficacious drug therapy for our patients. Review, process and monitor physician orders and patient medication profiles. Responsibilities include providing pharmacy information to physicians and nursing, providing pharmacotherapy to our patients, and patient safety. The Pharmacist is responsible for contacting other health care professionals as appropriate and to being an active member of the health care team. Works within institutional and departmental policies and procedures and within the scope of professional practice standards and state and federal law. All functions are performed in keeping with continuous quality improvement standards. Performs duties and responsibilities in a manner consistent with our mission and values.

Principle Accountabilities: Pediatric Pharmacists

  • To promote the optimal use of medications (including prevention of improper or uncontrolled use of medications).
  • To educate healthcare professionals, the patients and the public in pharmacotherapy.
  • Order laboratory and other tests to help determine efficacy and toxicity of medication therapy.
  • Interview and evaluate patients upon admission to the hospital.
  • Design a pharmaceutical care plan as appropriate and document the plan in the patient's electronic medical record.
  • Process and/or insure the processing of all discharge medications for the assigned patient care area.
  • Counseling patients on drug therapies
  • Reduction of hospitalizations and ED visits, avoid polypharmacy issues, drug interactions, assistance w/medication
  • Provide appropriate recommendations to providers regarding anti-thrombotic therapy for inpatients.
  • Coordinate the transfer of patients to the Outpatient Coumadin Clinic.
  • Provides consultative services to all professional staff regarding drug information and therapy.
  • Coordinate with the Antimicrobial Stewardship Clinical Pharmacist Specialist to ensure appropriate antibiotic use.
  • Reviews, interprets and evaluates medication orders to identify, prevent and resolve potential or actual drug related problems.
  • Provides drug information and education to medical staff and patients.
  • Documents interventions and their outcomes for use in drug therapy quality improvement efforts.
  • Prepares and dispenses medications using appropriate techniques and applicable policies and procedures
  • Researches and organizes patient-specific information. Documents patient medication histories.
  • Recommends or reviews laboratory tests. Identifies medication therapy problems as appropriate
  • Follows up consistently on drug information recommendations and assesses effectiveness
  • Participates in monitoring and improving relevant outcome measures and/or quality improvement indicators
  • Designs and recommends pharmacotherapeutic regimens and corresponding monitoring plans
  • Monitors data and modifies pharmacotherapeutic plans as necessary. Documents pharmaceutical care activities in patients' medical records or clinical information system.
  • Perform other duties as assigned

  • Licensure requirements: Must have valid, open and active California Pharmacist license
  • Education: Pharm D
  • Years of experience: 1 + year(s) of recent pharmacy experience within a hospital setting. Must have Pediatric Experience
  • Schedule: Monday - Friday, may include rotating weekends
  • Length of Assignment: On going Per Diem
  • Pay: Depending on Experience
  • Background/drug requirements: Ability to successfully complete background check, client health screening criteria and drug screen
