QUALIFICATIONS:

  • Required: 2 - 3 Years - Experience in an in office level health care setting, MSW, LPN, or CNA 2 with experience managing a seniors care.
  • Required: Can navigate windows easily and learn new programs quickly. Must be self-sufficient in handling complex computing tasks.
  • Required: Excellent Driving Record and Background Record.


PRIMARY RESPONSIBILITES

  • Create and maintain client care plans daily with emphasis on creating high quality care plans and developing of extraordinary relationships with client and family members.
  • Meet with potential clients and family members to discuss their needs and provide solutions
  • Maintain ongoing communication and a consistent follow-up schedule with potential clients and families who have yet to commit to service.
  • Conduct ongoing in home assessments and quality care visits. Update care plans as needed based on outcomes
  • Field caregiver calls about client needs and or concerns and provide consistent follow-up and follow through
  • Work with other team members to coordinate various aspects of a client's care.
  • Conduct client and CAREGiver introductions
  • Demonstrate open and effective communication with clients, family members, the franchise owner, colleagues and CAREGivers, and referral providers/care providers.
  • Develop and maintain positive and professional relationships with referral sources in the community.
  • Recognize and capture opportunities to increase service hours in scenarios to enhance and/or increase quality care.
  • Excellent Customer Service: You truly believe the customer is always right and will do all measures to ensure their satisfaction.
  • Manage conflict. Must not be afraid to make the tough calls and follow through to a quality closure. Conflict Management is required
  • Fast, efficient, and creative in solving problems relating to people and logistics.
  • Must be extremely flexible and able to "go with the flow" or change direction in a very fast changing environment
  • Daily planning and prioritizing client and office needs using fluid and detailed organizational skills
  • Maintain a high energy level with the ability to multi-task with a high level of accuracy at a very fast pace
  • Daily driving to and from clients homes and the office

· Monday through Friday 8-5pm
· On-call as needed and rotating weekends
