A Nurse Navigator facilitates patient follow-up through patient coaching, information transfer, and communication in order to improve continuity of care, enhance the medical management of complex disease, and streamline the intervention processes include: * Serve as the hospital ambassador providing patient support from initial contact throughout the continuum of care.
* Instructs and educates patients and families on primary diagnosis.
* Assesses and coordinates patient's discharge planning needs with members of the healthcare team.
* Provide support to patients with potential issues relating to: insurance, transportation, language barriers & discharge planning.
* Meet with target physicians in the community to discuss and promote the hospital services.
* Participate in community outreach activities to provide education. * Conduct follow up interviews with patients to assess their condition, determine needs and follow up on any outstanding concerns.
* Collaborate with Quality department to ensure that performance measures for the program are sustained in accordance with the goals for the program * Orients and mentors new staff members.
* Follows Standard Precautions using personal protective equipment as required.
* Florida RN license * BLS & ACLS * Minimum 1 year ER setting experience with direct patient care.
* Exceptional customer relations and interpersonal skills.
* Demonstrated experience relating to process improvement initiatives.
