LPN Case Manager - Full-time / Part-time

Aerotek is hiring an LPN Case Manager for one of our clients in Brentwood.

Job Responsibilities:

• This individual will be responsible for assisting medicaid members with their chronic illnesses telephonically.
• They will be working with the member to develop a care plan and discuss goals.
• They will be responsible for carrying a certain case load and meeting quality audit standards.
• It is preferable this individual have 1+ year(s) of health plan experience and at least 1+ year(s) of hands-on, clinical experience.
• It is required to have a compact license since members will be in another state.

Hours: Monday - Friday; 8:00am - 5:00pm
