The Stroke Program Manager, working collaboratively with theMedical Director and the Market Quality Director, is responsible for all operational aspects and evaluation of the Stroke Program including program development, planning, organizing, implementation, assessment, patient flow and performance improvement.
The Program Manager is responsible for assuring all stroke program standards are met as required by CMS, The Joint Commission and the American Stroke Association Get with the Guidelines program.
The Program Manager coordinates stroke certification/recertification, and, in collaboration with Quality, reviews medical records for compliance to all standards and develops and implements action plans based on analysis of data.
In collaboration with the multi-disciplinary team, is responsible for developing systems to enhance coordination of care across the continuum, community and EMS outreach and education, staff, physician and patient education and marketing communications.
The role includes translating research into practice through the management of clinical pathways, order sets and other resources to support the program; acting as a resource in stroke clinical specialty to colleagues across disciplines; serving as a role model/leader in process improvement initiatives for stroke care. She/he will function as a role model in the use of the nursing process and develop, introduce and evaluate new approaches to stroke nursing practice.
Assures compliance with Stroke Program standards (CMS, GWTG, TJC) Coordinates stroke certification and recertification.
Develops systems to enhance coordination of care across the continuum.
Provides direction to the multidisciplinary team on community outreach, education and marketing.
Translates research into practice.
Serves as resource in stroke clinical specialty Licensure/Certification Required Registration, Licensure and/or Certification * Current Licensure in the State of Kansas as an Advanced practice nurse (Nurse Practitioner) * Current Basic Life Support (BLS) verification course, asspecified in policy * Successful completion of Dysrhythmia Exam within allotted time frame, as specified in policy * Advanced Cardiac Life Support (ACLS) verification course within allotted time frame, as specifiedin policy * National Institutes of Health (NIH) Stroke Scale certification Preferred Registration, Licensure and/or Certification * HealthCare Quality certification * Critical Care Experience Education Required Education * Master's degree in Nursing Preferred Education * Master's degree in a healthcare-related field Experience Required Experience * Minimum of 5 years healthcare experience * Minimum of 3 years experience demonstrated involvement in management of stroke patients and a solid grasp of hemodynamics * Supervisory and /or management experience Preferred Experience * 5 years clinical health care experience Required Knowledge and Skills * Comfortable and skilled at working with physician, health care providers and other stakeholdersin the organization * Capable of gaining immediate credibility with individuals throughexperience, presentation, communication skills, empathy and compassion * Analytical skills includinga working knowledgeof basic statisticsand statistical analysis methodologies * Knowledge of PC based computer software, i .e.
Word and Excel * Ability to work independently and interdependently * Broad knowledge and familiarity with healthcare-related regulatory and accreditation requirements * Extensive knowledge of Regulatory Compliance standards of TJC, CMS, and other regulatory agency standardsaspertinent * Knowledge of positive process change strategies, Quality Management and Performance Improvement principles * Experience with continuous compliance processes
