Job Description

Job Summary:

The Clinical Nurse Navigator RN serves as a patient navigator throughout the continuum of care.

Responsibilities:

  • Functions as a liaison for management staff, administration, physicians, managed care companies, community organizations, and other customers.
  • Coordinates efforts for treatment and promotes quality improvement and educational efforts.
  • Compiles data, tracks outcomes, and makes recommendations for process improvement.
  • Provides referrals/support for follow-up care for patients who are uninsured or underinsured.
  • Integrates patient-centered care in the nursing processes to include the patient(s) and family in care decisions, incorporating evidence based practices to achieve safe and effective patient and process outcomes.
  • Provides education to patient/families at their level of understanding, when and where needed, in a culturally and linguistically appropriate manner, to engage them in self-care management for their highest/optimal level of health.
  • Advocates for the unique needs of individuals and the population served: anticipates and removes barriers, and acts to assure service(s) access and service effectiveness to protect the vulnerable.
  • Provides guidance and planning to prepare patients and families for future care needs, ensures continuous access to services, and actively promotes patient engagement to adopt and achieve their self-care goals.
  • Performs other duties as assigned.

  • Qualifications

    Licenses/Certifications/Registration:

  • Required Credential(s):Licensed Registered Nurse credentialed from the Texas Board of Nursing.
  • Education:

  • Bachelor of Science Bachelor’s degree in Nursing is required. Three (3) years of nursing experience in complex or chronic disease management or acute care is required. Experience in case management and/or outpatient care is preferred. Experience in managing complex chronic disease patients within the community setting across the continuum of care preferred.
