PRIMARY FUNCTIONS:  Provide support to the Care Managers (CM), Care Coordinators (CC), and Engagement Services Staff (ESS) within the Care Coordination Department in the implementation of core services to Health Home Program clients.   SPECIFIC DUTIES:        Assist CMs, CCs, and ESS with client appointments and accessing needed services as identified on client's care plans.       Participate in the implementation of plans to the satisfaction of the clients served.       Support Care Manager in providing core services for client monthly, as required or when needed, to assure all care plan goals are addressed.       Support Care Manager in ensuring adequate coordination, appropriate communication and maximum cooperation between all sources of support and services.       Provide support to clients in completing applications and paperwork for and management of benefits, financial, housing, or other related areas as needed.       Assist in Outreach and Engagement (O&E) activities such as distributing program brochures, locating eligible clients to build caseload, maintain contact with potential referral sources and completing intake paperwork as applicable.       Assist clients “lost to care” and maintain ongoing contact with enrolled clients, as requested.       May maintain a temporary case load when needed.       Maintain accurate and verifiable documentation of all services rendered.    Maintain telephone availability for clients during the workday.    Provide crisis intervention and follow-up.    Serve as client advocate.    Participate in staff meetings and other Care Coordination department activities.    Ensure compliance with all applicable local, State, Federal, and Health Home Network regulations, and agency policy.    Participate in all mandatory training.    Represent Catholic Charities Community Services on appropriate boards and committees as assigned by Care Coordination Director.    Other duties as assigned.   QUALIFICATIONS:             High School Diploma/GED and two years of human service experience.             OR             Associates Degree in Health or Human Services and one year of human service experience.             Bilingual (English/Spanish) is required for some positions.             If position required bilingual skills to be used, additional compensation will be offered.   ADDITIONAL REQUIREMENTS:       Demonstrate ethical business practices, in conformance with all state and federal laws and regulations.      Possess excellent verbal and written communication skills.      Previous experience working with diverse populations, including low to moderate-income families.      Ability to multi-task and prioritize duties.      Ability to travel in all weather conditions throughout the counties served.      Willingness to foster agency, department and program wide cooperation and team work through use of positive/constructive communication techniques.      Ability to maintain absolute confidentiality in regards to all records reviewed including consumer records, employee records and billing records.        Proficiency and experience with PC's and Microsoft applications.      Ability to analyze and interpret data and to handle problem resolution.      Possession of a valid NYS Driver's license and a driver's record considered acceptable by agency and insurance carrier.      Continuous use of a reliable, registered and insured vehicle.      Demonstrate full adherence to the CCCS Code of Conduct and all policies/procedures related to compliance.      Demonstrate commitment to Agency Mission Statement.   Ability to meet the following physical requirements with or without reasonable accommodation:    Use hands to manipulate, handle, feel, and control items or equipment, including motor vehicle;   Stand, sit, walk, bend, stoop, kneel, and reach;   Climb up or down stairs;   Able to reach above or below shoulders;   Occasionally lift or move objects weighing up to 10 lbs;   Sitting at a desk or in a vehicle for long periods of time to perform certain job functions;   Be able to read write and interpret written documents.Democrat and Chronicle.
