This is a professional nursing position responsible for comprehensive, family centered assessment, care planning, care plan implementation/service coordination, and outcome measurement for medically complex children, birth to 21 years of age, with multiple medical diagnoses. Professionals occupying this position may carry caseloads between 150-200 patients. This position reports to the Senior Registered Nurse Supervisor.

This position works Monday through Friday; no nights; no weekends; no holidays. Flexible work schedule is an option.

Responsibilities include, but are not limited to:

Conducts nursing assessments of children with multiple diagnoses and medically complex conditions to include assessments of family strengths, resources, and needs.

Accepts referrals for CMS services and screens them for eligibility. Participates in the application and funding authorization process.

Coordinates development of Care Coordination Plan with parent/primary caretaker and other disciplines; assumes responsibility as lead service coordinator when appropriate; assure updates and appropriate distribution of plan.

Conducts patient/primary caretaker teaching about medical condition and prescribed treatment and counseling about chronic grief and coping with chronic illness; provides information about maximizing growth and development and accessing all community resources.

Continuously assesses community resources and identifies gaps in services; uses innovative and creative strategies for development of needed resources.

Regularly monitors and documents patient's/family's progress toward Care Coordination Plan goal achievement and develops creative/innovative strategies to assist in goal achievement.

Coordinates appropriate utilization of expanded well check services.

Participates in multidisciplinary, comprehensive, subspecialty clinics to include: review of patient's medical record to assure documentation of all interventions and consistency with plan of care; completion of updated nursing history including current medications and immunization status; assessment of current strengths, needs, and condition; participation with physician in physical examination and appropriate well check screening; post-clinic counseling appropriate to the patient's/primary caretaker's needs, developmental level, and unique circumstances.

Conducts quality improvement activities.

Maintains current knowledge of pediatric community health practice and provides in-services training to co-workers, community agencies, parent support groups and providers to enhance awareness of children's health issues; professionally represents the program on community advisory committees, task forces, work groups, etc. as requested by Nurse Supervisor or Nurse Director.

Performs other related duties as requested.
