About this job
We are looking for an experienced RN to join our team of professionals as an RN - Client Care Coordinator in our Harrison office. The Client Care Coordinators perform a variety of duties including care consultations with potential clients and family members, client/caregiver introductions and quality assurance visits with existing clients. They determine each individual client's needs to provide solutions and create tailored service plans. They continuously evaluate the plan through regular communications and visits to ensure high quality care and client satisfaction. Experience with AR Medicaid and Targeted Case Management is a plus.
