JOB DESCRIPTION

This is a professional nursing position responsible for Specialty Clinic Nursing and Nursing Care Coordination for children with severe and chronic illnesses. The professional occupying this position shall be assigned to specific clinic specialty areas to provide assistance to the Physicians in the clinic; to offer support and teaching to parents; to assure that each child receives optimal care. The professional may also be assigned to assist clients with psychosocial needs.

This position reports to the Senior Registered Nurse Supervisor. Care Coordination activities are targeted to champion, excellence in the delivery of health care for children with special health care needs. The professional exhibits effective communication, courtesy, respect and understanding when dealing with clients, the public, coworkers and staff of other community agencies. Responsibilities include but not limited to:

Conduct Care Coordination assessments of children with special health care needs. Coordinate and document in PM/CC services for the children/families, including multiprogram staffing as indicated and follow-up on service provision.

Monitor the child's/family's progress toward accomplishing treatment goals.

Assess community resources and identify gaps in services, in order to coordinate patient/family needs.

Provides education for child's care, procedures, and child development to patient/parents in clinic and out of office setting, if applicable.

Provides supportive counseling and guidance to patient and family. Refer to appropriate resources, as needed.

Authorizes purchase of prescribed patient services after review of physician's assessment and treatment plan.

In conjunction with physician and social worker, evaluate need for protective action for abuse/neglected children. Referral is made to appropriate agency for follow up.

Administer vaccine safely following protocols and procedures for clinic. The storage of vaccine in appropriate area and temperature must be maintained.

Perform clinical duties as assigned and provide related care coordination services as a result of those clinical activities.

Documentation utilizing EHR and/or PM/CC system of medical record accurately & update system as necessary.

Employee will be in compliance with practice guidelines including preventative health care, continuity and coordination of care and Hedis performance measures.

Performs other related duties as required, so that the total needs of the child and family are met.

EXAMPLES OF WORK

Provides care coordination to children with severe and chronic health problems with attention to physical, developmental and psychosocial rehabilitation needs.

Provides accurate documentation of care coordination activities in client's electronic record according to CMS Performance Standards.

Completes periodic assessments and updates.

Provides health education on a variety of topics including but not limited to Healthy Behaviors, Chronic Disease, well child checkups, immunizations, dental care, disaster planning, transition planning, etc.

Interviews CMS enrolled clients and/or families. Completes eligibility screening, initial and ongoing assessments and care plans and education as per Children's Medical Services Performance Standards. Explains policies and procedures. Completes necessary documentation.

Serves as a community resource person in the field of pediatrics. Coordinates patient services with other state and local health agencies or providers to avoid duplication of services and to provide information to clients and other health care professionals. Participates in the Medical Home Initiative.

Performs clinic functions as assigned, assuring case records are complete and accessible in the clinic, performing nursing histories and assessments. Conducts pre and post interviews with all clients and families during which the nurse reinforces MD orders, provides teaching and emotional support and documents outcomes.

Attends staff meetings, community meetings, and continuing education programs for professional development and licensure.

Responsible for participating in and responding to Emergency Operations and Disaster Recovery efforts as events arise within assigned office, region or state.

LICENSURE, CERTIFICATION OR REGISTRATION REQUIREMENTS

Licensure as a Registered Professional Nurse in accordance with Florida Statute 464 or eligible to practice nursing in accordance with Florida Administrative Code 210-8.27 and three years of professional nursing, psychiatric nursing or nursing instruction experience. A bachelor's degree from an accredited college or university in nursing or nursing education can substitute for one year of the required experience.
