Aerotek is hiring Telephone Triage Nurses for a client in Nashville.
Hours: Monday - Friday, 8:00am - 5:00pm
Responsibilities:
- Telephonic care coordination with palliative care patients and their families, including triaging incoming calls as well as making proactive outgoing calls as requested by Nurse Practitioners
- Assess, identify and close clinical and non-clinical gaps in patient care
- Appropriate triage and escalation of routine, urgent and emergent patient calls
- Clinical care coordination with PCP's and other providers and specialists, including
- PCP or specialist office visit appointment coordination
- Initiating health plan authorization for Home Health, DME, Home Infusion and other critical services. Verifying the timely delivery of these services.
- Medication management, including identifying potential medication issues, answering patient questions on side effects and medication conflicts, and coordinating refills.
- Identifying community resources and assisting patients and families in accessing those resources
- Referral to social worker and chaplain resources when appropriate.
- Provide comfort and reassurance to patients and family members
- Assist Aspire nurse practitioners in managing their calendars when needed to manage urgent and emergent home visit needs
- Various clinical and administrative tasks
